I have found several that are similar but none have flat tops and bottoms.  Help?

If this is the only sample, it could well be hand-drawn rather than typeset.

Given the crude geometric shapes, my suggestion would be to fontstruct your own.

It's not actually as crude looking as it seems but, you maybe correct.

P.S.  Forgot to mention that I tried Fontstruct the other day.  The top 1/3 of the letters were missing in the saved file.

I agree with Ralf—the logo appears to be custom/hand drawn.

Forget Fontstruct—import the first image you posted into Illustrator and auto trace it—the shapes are simple enough that you should get a decent recreation.
