I think that CHORUS is an Avant-Garde-Gothic-Book-Outline-Yourself with a twist for the C. And I bet that the symbol is not part of the font.

Agree with Ignacio that the typeface for CHORUS is derived from Avant Garde, but this particular iteration is called The Maple Origins (and yes, the treble clef symbol is an addition).
